Social Prescribing Link Worker

In this role you will work as part of a small team of Social Prescribers providing 1 to 1 personalised support to people referred from the SWIFT Primary Care Network (PCN) area (in South Bristol).     We have a long experience of delivering social prescribing services in South Bristol.

This post empowers people to take control of their health and wellbeing by giving time to focus on ‘what matters to me’. You will build trusting relationships with people, create a shared personalised care and support plan and connect people to community groups, other voluntary and social enterprise organisations and services.    You will also work with partners to provide support to community groups and Voluntary, Community and Social Enterprise organisations (VCSE) involved in social prescribing.

This role helps people to work on their health and wellbeing, specifically wider determinants of their health, such as debt, physical inactivity, as well as other lifestyle issues and low-level mental health concerns, by increasing people’s active involvement with their local communities. This approach particularly helps with long term conditions (including support for mental health), people who are lonely or isolated, or who have complex social needs which affect their wellbeing.

You must be a good listener, have time for people and be committed to building sustainable local communities that care for each other.   You will have experience of working positively with people facing complex social and emotional challenges.   You will have great interpersonal skills and able to get along easily with people of all backgrounds.   If you are dedicated to improving the wellbeing of people and their communities, we would love to hear from you.

About our Team:

We are a small very friendly and supportive team, with a breadth of community knowledge.    As an organisation, promoting good health and wellbeing, we take care to support each other and offer regular supervision in work roles. We are based at the Knowle West Healthy Living Centre on Knowle West Health Park.

Volunteering Opportunities

 

Hi there! Have you been looking for a way to occupy your spare time, engage with other locals, give back to the community or just feel useful? Come and volunteer with us at the Healthy Living Centre,  we’ve a range of opportunities and the most frequent of those are listed below.

Volunteers will be asked to fill in a simple form and to complete DBS checks, to ensure they are suitable for work with vulnerable adults and children. You will also be asked to complete some basic training which will give you new skills to add to your CV. In return out of pocket expenses and on the job supervision and support will be given.

    The Board of Directors continually needs local people to become involved. Training and support will be offered, the time commitment will be a minimum of 3 hours per month. The Board may from time to time need to recruit individuals with certain expertise, at other times we may need only general local knowledge.
